Privacy Rights Request Form

Please use this form to submit a privacy-related request to Maxwell Locke & Ritter LLP (“ML&R”), including requests to access, correct, or delete your personal information; opt out of the sale or sharing of your personal information or its use for targeted advertising; or limit certain uses or disclosures of your sensitive personal information, as applicable.

ML&R will endeavor to respond to your request promptly and in accordance with applicable law. In general, we will endeavor to respond within 45 calendar days from the date we receive your request. If additional time is needed, we will notify you.

Please note that certain circumstances may limit our ability to fulfill all or part of your request. In limited circumstances permitted by law, we may also charge a reasonable fee for responding to a request.

If we need additional information to verify your identity or process your request, we will contact you. Any information collected for verification purposes will be used solely to verify and process your request.
